CAYA is proud to announce that a new resource has been added to its website, a MAiD questionnaire that is customized for British Columbians and the law as it currently stands in this province.
This questionnaire was developed by Taucha Post, a former SLP at CAYA, and Nicolas Baldwin, MD.
From the document:
“This is an adapted questionnaire for non-verbal patients requesting MAiD. This tool is intended to provide non-speaking patients with the opportunity to express their wishes and rationales as accurately as their speaking peers as possible.”
“This document was developed to supplement the Medical Assistance in Dying Assessment Record Form from the British Columbia Ministry of Health. Prescribers can use their discretion when selecting sections and items to administer.”
A copy of the questionnaire is available for download in our Resources section.
